In addi-tion, MRI/MRCP is very sensitive for identifying whether a patient has single or multiple PCN, with the latter favouring a diagnosis of multifocal side-branch IPMN.25 33 36 37 Patients with PCN may Also, MRCP may be used to diagnose other conditions including a type of pancreatic cyst called an intraductal papillary mucinous neoplasm (IPMN). How Should Patients Prepare for an MRI or MRCP Scan? Before an MRI or MRCP, patients can eat and take medicines as normal, unless told otherwise. Although it is unclear what proportion of these incidentally-discovered pancreatic cysts represents IPMN, surgical series have shown that most of the larger ones indeed are branch duct IPMN (BD-IPMN).

Intraductal papillary mucinous neoplasms are tumors that grow within the pancreatic ducts (the pancreatic ducts are the "tubes" within the pancreas that are used to transport fluids to the bowel to help with digestion). Intraductal papillary mucinous neoplasms are also characterized by the production of thick fluid, or "mucin", by the tumor cells. IPMNs are important because some of them progress to invasive cancer if they are left untreated. Magnetic resonance cholangiopancreatography (MRCP) is a special type of MRI that uses computer software to image the pancreatic and bile ducts, areas where tumors often form. MRCP is also used to see pancreatic cysts and blockages in the ducts.

• Cystic lesions of the pancreas occur in 2.5% of the population (increasing prevalence with age) • Many of these lesions will represent pre-cancerous lesions: IPMN and MCN • IPMN represents a whole gland process with several genetic pathways to distinct forms of invasive cancer • Resection should be recommended when there is concern for high-grade dysplasia Main duct IPMN Large branch duct lesions with mural nodules The size of the IPMN and the diameter of the main pancreatic duct associated with incidence of IPMN-derived carcinoma (SHR 1.85; 95% confidence interval 1.38-2.48 for a 10-mm increase in the IPMN An MRCP is a non-invasive test that uses a strong magnetic field to view the liver, pancreas, gallbladder, and bile ducts. This test can show if the bile ducts are obstructed, such as by a suspected IPMN. Contrast dye, given either via a drink or an IV, may be used during this test to help enhance the images. Intraductal papillary mucinous neoplasms (IPMNs) are some tumor structures that are developed in ducts of pancreas composed of mucin-producing columnar cells .

MRI based on its superior contrast resolution for recognition of septum, nodules, and duct communication, can be a procedure of choice for evaluation of a pancreatic cyst, as has been suggested by a consensus of radiologists. Intraductal papillary mucinous tumor (IPMT) of the pancreas is characterized by the presence of mucin-producing tumor and cystic dilation of the branches of the pancreatic duct in the uncinate process (branch duct type), diffuse or segmental dilatation of the main pancreatic duct (main duct type), or dilatation of the main duct and the branch ducts (combined type) (, 1)– (, 3) (,,,, Fig 1).

my friend was diagnosed with ipmn in her pancreas. there are several cysts and lesions but no cancer. what would happen if one of her lesions or cyst were to leak. she is now running a fever everyday and feeling sick all over.
